Whooping cough (pertussis) has been widespread in California. Students throughout the state have missed school because they were sick, and some babies and children have been very ill with pertussis. To help protect your children and others from whooping cough, a new California law now requires students to be vaccinated against whooping cough.
This form must be returned to your school prior to the start of school next fall.
However, you are encouraged to have your child receive the booster as soon as possible, and to mail or bring certification to school as soon as you have the doctor fill it out.
